Last minute changes by smart people result in much more subtle errors.

Indeed. Smart people who think they couldn't possibly make mistakes can be quite dangerous.

Work stories: a place where I worked was moving to a new building. Due to budget cuts, the heights of the floors were reduced to save on concrete, gas lines were left out, ditto soundproofing. These changes were made during construction.

I no longer work there, but last time I asked, they were still dealing with the consequences of those decisions.

Luckily the abovementionned contractors are computer programmers and not construction people. I'd refuse to step into any building they might have been involved in putting together.
