My team lead came up with a new excuse after I pointed out how today's 'problem' was his fault, not mine. Sure, most of his email was a non-apology invoking communication failures. Nothing new there. He then said that some of those failures came from his being somewhat distracted after he sliced his fingers when he opened a tin can.
